Team Registration and Team Roster Form

Member Centre's will be registering their division teams on Thursday April 14th at the New Lowell Legion between 7:30 pm to 9:00pm. Please have all the required information ready for our registar.

This year Deanna Hunter will be SSMBA's 2011 Registar. To help Deanna out, listed below is the forms which may be downloaded and filled out prior to April 14th.

After you fill out the forms on your computer please print a copy for yourself as well as email a copy back to Deanna Hunter as soon as possible.

To help out Deanna for the insurance please click on the link below and after it opens simply choose "File" from the menu on top then choose "Print".
Now fill it out and bring it to the meeting Thursday April 14th. You do not need to fill it out online and send a payment.
